Identify The Data And Service
Start with the account, app, platform, service or contract from which data is sought. Identify whether the data was provided by the individual and whether it is processed by automated means.
Explain the requested format and whether the individual wants the data sent to them or transmitted to another controller.

- Account, customer, app or service records identifying the data.
- Categories of personal data requested and why they are said to be provided data.

Show The Response And Practical Problem
Include the organisation's response, any identity checks, time extension, refusal reason or technical explanation. Keep the portability issue separate from subject access, erasure or objection unless those requests were also made.
If another person's data is involved, identify that issue clearly and include any correspondence about consent, redaction or joint accounts.
- Controller response, refusal, delay or partial data export.
- Identity-check correspondence and any fee or manifestly unfounded assertion.
- ICO complaint, case reference and outcome if started.
- Evidence of loss, disruption, switching difficulty or inability to reuse the data.
